All claims must be post marked no later than December 31, 2011 to be eligible for payment. All Body Armor 4x4 products are eligible for the rebate, purchases are not limited to just one item, you can purchase multiple items in multiple transaxtions to earn the maximum rebate level desired.
COMPLETE REBATE DETAILS:
1. Retain copy of the original receipt of purchase
2. Completed Body Armor 4x4 claim form
3. UPC Product labels from each item is required!
4. Claim forms and Proof of purchase must be postmarked no later than December 31, 2011.
5. If purchasing more than one Body Armor item over the length of the promotion, save your receipts and submit all at one time in order to receive the maximum rebate level.
6. Rebate is based on the purchase price of the Body Armor product as shown on the receipt, freight, taxes or other charges will not be considered
